Sexual Harassment Complaint
Reports may be submitted anonymously. Reporting anonymously means you disclose what you believe you have experienced (or are aware of an incident involving) sexual harassment or sexual violence without identifying yourself or providing sufficient information to determine your identity or request any specific action. You may choose to make an anonymous report if you want someone at Laurel to be aware of your experience, but do not want to be involved in an administrative investigation. Please note, however, that Laurel may be limited in its ability to investigate or respond to the incident if it does not have sufficient information from which to follow up on such a report. Items marked with a (*) must be completed for submission. Items not marked are not required.

- You have the right to file a report with the Police as well as to the school. You can file a criminal complaint with the local law enforcement agency, depending on where the incident took place.

Act 16 of 2019 requires postsecondary institutions to establish an online reporting system for complaints of sexual harassment and sexual violence — from students and employees — including those submitted anonymously.
This form can be used by students, employees, and third parties to file a complaint of discrimination based on race, color, national origin, religion, creed, age, disability, sex, gender identity, sexual orientation, familial status, pregnancy, predisposing genetic characteristics, military status, domestic violence victim status, or criminal conviction.
Please note that submissions using this form may not be reviewed outside of normal business hours. If your immediate safety is at risk, if you are witnessing violence, or if you perceive imminent harm to yourself or others, please immediately dial 911.
Questions
The Campus Director will have access to reports, statements made by the Complainant, Respondent, witness, and any other materials gathered during the investigation.
How will the reported information be used?
After submitting this form, school administration will evaluate the report and take appropriate steps to respond in a manner that is timely, supportive and fair. It is important to note that these procedures provide individuals who have experienced an incident of sexual harassment or assault with the ability to provide input and maintain a large degree of control over how their issue is handled at every point in the process.
Can I submit this form anonymously?
Yes, this form may be submitted anonymously. The school takes all reports of discrimination and sexual harassment seriously, but its ability to resolve an issue will be limited if it is reported anonymously. However, responsible employees (all school faculty and staff) are required to include their name and contact information when completing the form.
Is the information supplied in this form confidential?
While the school will endeavor to maintain the privacy of reporters, consistent with school policy and the law, details of a complaint may need to be shared (including the identity of the reporter) in certain circumstances for the school to take the appropriate action.
